Each major has a program of study, or check sheet of course requirements. Students use this form along with their Program Evaluation from MySuffolk to keep track of their degree requirements.

Multi-Year Planner

A multi-year planner form is a simple grid to help you plot out your degree plans. Over time, and with the help of faculty and advisors, every student will learn to create a customized plan that graduates them from Suffolk in a timely manner while also incorporating study abroad; clubs and organizations; as well as internships.

Change of Major, Minor or Advisor

Use this form to declare or change your major/minor or to request a change in your advisor. All major requirements may be found under the specific major/departmental web pages.

Incomplete Contract

Use this form to document the remaining requirements in a particular course when both the instructor and student agree that an "Incomplete" grade is warranted. Incompletes are only authorized in certain situations. Be sure to review the undergraduate catalog for current information.

Incomplete Extension

Use this form to request an extension of your incomplete deadline. Once your deadline passes, incomplete grades automatically turn to an "F."
