It is the student’s responsibility to initiate the request for accommodation(s) and adhere to university policies and procedures. The student is responsible for maintaining active communication with both faculty and the Dean of Students (DOS) Office throughout the semester.

The following is a checklist to assist in arranging for extended time testing:

What are your responsibilities as a student?
  1. Pick up accommodation letter(s) from DOS Office
  2. Hand-deliver letter to your instructor (preferably within the first two weeks of the semester)
  3. Meet with your instructor to discuss accommodations
  4. Initiate testing arrangements with your instructor at least one week prior to the exam date
  5. Confirm arrangements with instructor 1-2 days before exam
  6. If testing outside the classroom, enter the testing area with only those materials specified by instructor

Please Note: Students receiving extended time are expected to uphold the same University standards of honesty and preserve the academic integrity of the exam

What are your responsibilities as a faculty member?
  1. Arrange for testing accommodations and communicate clearly with student prior to the day of exam
  2. Contact the Ballotti Learning Center (573-8235) and request a Faculty Extended Time Exam Instruction form
  3. Fill out the form completely to ensure the exam is administered correctly (please indicate clearly any special instructions for the exam)
  4. Send the completed form at least three days prior to the exam date to Tim Brown, BLC, Donahue 208
  5. Deliver the exam to the Ballotti Learning Center at least one day prior to the scheduled exam (failure to provide advanced notice may result in loss of testing space)
  6. Be sure to indicate on the form the preferred method for the exam to be returned to you

Please Note: Instructors may also provide this accommodation by allowing a student extended time directly before or immediately after a scheduled exam time, during office hours, or at any other mutually agreed upon time and location
