Dear Students:

If you wish to request your accommodation letters please fill out and submit the Request form below.

Your request will be sent to the Office of Disability Services. You may be contacted if further information is needed.

You will be notified when your letters are ready and at that time you may pick them up at the Office of Disability Services.

Please note: Information submitted via this form is not sent over a secure line. Please visit the University's Internet Privacy Policy for more information. If you do not wish to submit the Request form online please call the Office of Disability Services at (617)994-6820 to submit your request over the phone.
