For Scheduled Appointments:
Completing the necessary forms before arriving to your appointment will help to make your visit faster. Please print, complete, and bring the form(s) with you to your appointment
If you are more than 10 minutes late to your appointment, you may be asked to reschedule.
When you arrive to Health & Wellness Services, please make sure to check in with a staff member at the front desk.
Note: Failure to arrive 10-15 minutes early will result in a longer wait time. Arriving 15 minutes before your scheduled appointment (30 minutes for a first appointment) will ensure that everyone is seen in a timely manner. Having your completed paperwork ready will also aid in the process.
Make sure that you have your health insurance card and Suffolk ID.
For Walk-In:
Walk-in hours fill up quickly and is a first come first served process. A good rule of thumb for a Walk-In visit is if you are feeling ill or have an injury (sprains, cuts etc). Walk-In hours can also be used for time sensitive issues such as if you need emergency contraception or immunizations. Please feel free to call us with any questions.
Note: Wait time may vary, depending on how many people have signed up for walk-in slots before you.
