Payroll and Tax Withholding Forms

Each of the following forms can be used by faculty, staff, and student employees.

• Payroll direct deposit authorization
• Federal form W-4 certificate (federal tax withholding)
• Massachusetts M-4 certificate (Massachusetts tax withholding)

Each of the above should be completed, signed, and delivered to the Payroll Office through interoffice mail or in person.

University Credit Union

Suffolk University employees and their families are eligible for membership in the University Credit Union. For information on opening account, please contact the Credit Union at 617-739-7447.

Existing members can have funds deposited directly from their Suffolk payroll to their credit union account using the University Credit Union direct deposit authorization form.

The form should be completed, signed, and mailed or faxed to the Credit Union, who will then process it and forward it back to the Suffolk Payroll Office.
