• What To Do Before Your Appointment

For Scheduled Appointments:

Completing the necessary forms before arriving to your appointment will help to make your visit faster.  Please print, complete, and bring the form(s) with you to your appointment.

Health History & Immunization Form - This is for all students who will be making their first visit to Health & Wellness Services (this is the same form you received in your Orientation packet), and any student who has had a significant change in their health status since the last time they filled out this form. 

Note: If you have not downloaded and completed the form(s) listed above, please arrive to your appointment 10-15 minutes early.  This will ensure enough time for you to complete the form at Health Services.  If you are more than 10 minutes late to your appointment, you may be asked to reschedule.   

When you arrive to Health & Wellness Services, please make sure to check in with a staff member at the front desk.

Make sure that you have your health insurance card and Suffolk ID.

For Walk-In:

Walk-in hours fill up quickly and is a first come first served process. A good rule of thumb for a Walk-In visit is if you are feeling ill or have an injury (sprains, cuts etc). Walk-In hours can also be used for time sensitive issues such as if you need emergency contraception or immunizations. Please feel free to call us with any questions.
