Group Insurance Plans
Health Insurance
All employees who are regularly scheduled to work 21 or more hours per week and all titled faculty (professor, associate professor, assistant professor, instructor) are eligible for membership in one of the Group Health Insurance Plans, effective with their date of employment. Lecturers should consult their collective bargaining agreement [PDF]. The employee and the University both contribute to the cost of health insurance. Benefits are available for spouses, domestic partners and dependents.

Dental Insurance
All employees who are regularly scheduled to work 21 or more hours per week and all titled faculty (professor, associate professor, assistant professor, instructor) are eligible for membership in the Dental Insurance Plan. Benefits are available for spouses, domestic partners and dependent children. Lecturers should consult their collective bargaining agreement [PDF]. Delta Dental Plan
Delta Premier Plan benefits are most comprehensive when members use a participating dentist. The plan provides full coverage for preventive and diagnostic services, 80% coverage for restorative services, and 50% for major services (e.g., crowns, inlays, and bridges) up to a calendar year maximum of $1500 per person. There is a $50 per individual, $150 per family calendar year deductible. The orthodontia benefit pays 50% of covered orthodontia expenses, up to $1500 lifetime orthodontia maximum per participant under the age of 19. Life Insurance
Basic Group Term Life and Accidental Death and Dismemberment Insurance
All administrative and support staff employees who are regularly scheduled to work 21 or more hours per week, and all titled faculty (professor, associate professor, assistant professor, instructor), are eligible for Basic Term Life and Accidental Death and Dismemberment Insurance, effective with their date of employment. Basic coverage equals one times the employee’s base annual salary up to $100,000, and is free of charge to the employee.
Additional Life Insurance
All administrative and support staff employees who are regularly scheduled to work 21 or more hours per week, and titled faculty (professor, associate professor, assistant professor, instructor) may purchase, through payroll deduction, additional life insurance equal to, at the employee’s option, 1 to 5 times base annual salary. The maximum amount of additional life insurance is $750,000. Medical certification is required for amounts over $375,000. The employee pays the full cost of the additional insurance which is calculated based on age and volume of insurance.
Long-Term Disability
Long term disability insurance (LTD) provides income protection when illness prevents you from working more than six months. You are eligible after being employed by the University for one year, or upon hire if you worked at another employer where the policy provided income benefits for at least 5 years.
You will receive 60% of your current monthly base pay up to a maximum of $15,000 per month, and any benefits received from the program are taxable.
You will continue to receive the benefit for as long as you qualify as “disabled” under the policy, up to the age limits as defined by the Certificate of Insurance. Your employment at the University will end after your sixth month of receiving long term disability benefits.
