*Supervisor may include Honors Coordinator, Club Advisor, Coach, CCE Staff Member, Internship supervisor, or other individual who can speak to whether the hours/responsibilities were completed.

Note to signer: The student must prepare a reflective description of experiences in this position, and must also compile a documentary archive of evidence of experiences in the position. You may request that the student share these materials with you before you sign this form.
