Academic Concentration - Notice of Enrollment

I hereby declare my enrollment in the concentration I am not currently enrolled in any other academic concentration. I have successfully completed my year (must have completed at least one year) and am in good standing. I have reviewed all of the elements necessary to complete the concentration, including the credit and grade requirements.

The latest date students may enroll in a Concentration is on or before (i) October 1st for students graduating the following January, and (ii) February 1st for students graduating the following May.
